Please fill out the request form and fax it to Lora @ 817-573-3646 or print it and bring it into the office.

Each request will be taken to the next Monday's staff meeting for approval and you will receive a phone call or an email with confirmation.

NOTE:  If you request an announcement in the bulletin or Journey,
          it will only be announced two times total.

          Childcare requests must have at least a two week notice.
          This is required in consideration for enlisting workers.
          Also, a pink "Childcare Sign-up List" must be completed.

          If you request the Kitchen, you must have someone present who has
          completed the Kitchen Use Training, or you will need to pay for
          Kitchen Staff. (See page 2 of request form for more details.)
